29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. In the Metal Fabrication Shop: The Accupress Press Brake was not guarded to prevent employees from having any part of their body in danger zones during operating cycles. Employees were exposed to amputation hazards. b. In the Metal Fabrication Shop: The guard on the Lodge and Shipley Shear to prevent employees from having any part of their body in danger zones during operating cycles was broken. Employees were exposed to amputation hazards.
