29 CFR 1910.178(l)(4)(ii)(B):Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a. 4050 Tremley Point Rd., Linden, NJ - Warehouse The employer did not provide refresher training for employees involved in an accident. An employee involved in a forklift accident returned to work and was operating equipment that included, but was not limited to Yale, model GLP030VX, and was not provided refresher training which included an evaluation of effectiveness of training prior to allowing the employee to operate the equipment. Violation occurred on or about June 22, 2021
