29 CFR 1910.147(d)(4)(i): Lockout or Tagout devices were not affixed to each energy-isolating device by authorized employees: The employer failed to protect employees from hazards associated with the unexpected release of energy. This was most recently documented on June 11, 2021. The extruder located on the 6th floor of B mill had steam running during servicing and maintenance activities. An alternative protection from exposure to steam was not provided to employees in the area.
