29 CFR 1910.147(d)(4)(iii)(A):Where tagout devices were used with energy isolating devices designed with the capability of being locked, the tag attachment was not fastened at the same point at which the lock would have been attached. On or about June 7, 2021, and at times prior thereto, six employees were tagging out equipment by placing tags on the toggle switches instead of on the energy isolating device, exposing the employees the hazard of being caught in the equipment.
