29 CFR 1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: (a) PHS 1 and PHS 4 Area: On or about 05/19/2021, and at times prior, the employer exposed employees to struck-by and caught-in hazards, in that periodic inspections of the energy control procedures were not conducted for lockout/ tagout (LOTO) including, but not limited to, PHS 1 Line, PHS 2 Line, and PHS 4 Line.
