29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): (a) On or about 4/15/2021-2746 County Road 59, Assembly Line #16, employees were exposed to a fall hazards of more than 6 feet from the roof to the floor in areas, such as but now limited to, a 18 foot by 11 inch dormer hole.
