29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 23, 2021, employees were exposed to a fall of approximately 8 feet while performing siding activities on a residential home located at 9170 Winfield, North Ridgeview, Ohio 44039. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.
