29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points. (a) In the SK room, at the head of the griller line #1, on or about 2/3/2021, employees were exposed to the unguarded nip point hazards of a rotating auger within a meat extruding device. Direct exposure to the nip point hazard resulted in the partial finger amputation of one employee. (b) In the Forming Line Room, on or about 3/23/2021, the delivery end of the product conveyor on the Nothum Superflex 24R breading machine, had an unguarded, ingoing nip point. (c) In the Packaging Room, on or about 3/23/2021, a Rovema Bagger VPS-260 bagging machine which had an unguarded ingoing nip point on the product delivery belt end. (d) In the Packaging Room, on or about 3/23/2021, employees were exposed to an unguarded pinch point on the product wrapping roller supported on a Rovema Bagger VPS-260 bagging machine (e) In the maintenance shop, on or about 3/23/2021, a Central Machinery drill press model #39955, serial #355851420 was not equipped with a shaft/chuck/bit guard, exposing employees to laceration, pinching, burning and caught in hazards created by point of operation, ingoing nip points, rotating parts up to 3,865 RPMs, flying chips and sparks. (f) In the maintenance shop, on or about 3/23/2021, a Grizzly Industrial mill/drill model #G0781, serial #1810731 was not equipped with a shaft/chuck/bit guard, exposing employees to laceration, pinching, burning and caught in hazards created by point of operation, ingoing nip points, rotating parts up to 2000 RPMs, flying chips and sparks.
