Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees working in the back store room using the Piqua Series Baler 40 were exposed to amputation, struck-by and caught-in hazards, in the instances listed below: A) The interlock system had been defeated to allow for the baler to run with the steel doors in an open position and not having to be securely locked, causing employees to be exposed to struck-by hazards. B) The gate had a hole in the mesh metal guard, approximate 5 inches in height and 4 inches in width, exposing employees to caught-in and amputation hazards.
