29 CFR 1910.178(a)(5): Employer requested that the PIV front-end attachments be modified with other than factory installed attachments but failed to request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about February 22, 2021, the powered industrial vehicle (Pole Hyster) used to raise, lower and stack rolls of carpet throughout the production area was found to have modifications to its carriage (forks replaced with lifting pole). The employer was unable to provide proof of the manufacturer 's approval to add modifications/additions, which affect the capacity and safe operation of powered industrial vehicle and did not add/revise forklift data plate for its inclusion.
