Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about February 11, 2021, the employer is failing to protect employees from struck-by hazards due to the unexpected release of stored energy. An employee was fatally injured while working under a Ready-mix concrete delivery truck that was not locked out to prevent the accidental release of stored energy. The truck rolled back and crushed the mechanic between the concrete floor and the axle wheel. Mechanics did not use wheel chocks to prevent the vehicle from rolling during servicing and maintenance.
