29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: Jobsite located at 63 Elm Street/158 Forest Street, Manchester, Connecticut: On, or about, February 11, 2021, employees working on the chimney structure for a cellular equipment upgrade project had not been trained sufficiently to inspect the worksite for deficiencies with the work platform provided and were not familiar with proper rigging procedures to ensure that personal fall arrest systems were properly designed and used.
