29 CFR 1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed: 8916 Shady Pond, Fair Oaks Ranch, TX. On or about January 16, 2021, and at times prior thereto, workers were exposed to fall hazards while using a damaged and modified A-frame ladder to scale in between toe boards placed on top of a steep pitched roof.
