Warehouse, Docks 3 and 5 - On or about January 8, 2021, the employer failed to ensure that each dock leveler was properly secured before they were driven over. The dock levelers at docks 3 and 5 were elevated and not flat, thereby exposing employees to hazards associated with uneven surfaces. b.)
