29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) 3301 Columbus Blvd., Philadelphia, PA - Maintenance Shop - The employer did not provide employees with training on usage and removal of energy control devices used to maintain the Yard Jockey. On or about November 17, 2021, while replacement of hydraulic cylinder on a fifth wheel of the Yard Jockey, an employee suffered crushing injuries when the employees arm was caught-in-between fifth wheel and the frame of a Yard Jockey.
