29 CFR 1910.303(b)(1)(iv):Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees such as damaged insulation. At the workplace located at 14200 Merritt Road, Grandview, Missouri 64030, an employee was exposed to a serious electrical hazard of 230 volts. The table saw that the employee was operating in the workplace had damaged electrical insulation near the power switch.
