29 CFR 1910.134(d)(1):The employer did not select and provide an appropriate respirator based on the respiratory hazard(s) to which the worker was exposed: Employee(s) engaged in an ammonia line break of Thermutator 1, while wearing full face cartridge respirators, were exposed to burn / inhalation hazards in the Taco room located at 512 S. Main Street, Hutchinson, Kansas. The full face respirators being worn by the two employees became saturated with ammonia gas during an unexpected ammonia release that engulfed the Taco room.
