Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.179(m)(1): A thorough inspection of all ropes were not being made at least once a month. There was no determination made as to whether further use of the rope would constitute a safety hazard. On or about 11/20/2020, Hitachi ABB did not ensure a thorough monthly wire rope inspection was made on crane JC-15, as evidenced by the wire rope failure on November 20, 2020, as well as subsequent inspection of both wire ropes after the accident, when multiple examples of deterioration was found, including broken wires per lay and strand, interior rust, and grease and dirt on the cables. Or, "In the alternative" OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck-by or fall hazards. On or about 11/20/2020, Hitachi ABB did not ensure a stacker crane, ID #JC-15, received visual rope inspections at the start of each shift.
