29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about September 21, 2020, at an excavation site located at the intersection of Lincoln and Santa Fe in Carney, OK, employees were allowed to work in a trench more than 5 feet deep that did not have a protective system in place, exposing the employees to struck-by and crushed-by hazards.
