29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about January 14, 2021, an employee performed maintenance and servicing on a Wash tank to remove the old electrical and motor controllers. Machine-specific procedures were not developed, documented and utilized for the control of potentially hazardous energy associated with the Wash tanks. Hazardous energy sources included, but were not limited to, electrical energy. b. On or about January 14, 2021, and at times prior, employees performed maintenance and servicing on the Harrop Kiln. Machine-specific procedures were not developed, documented and utilized for the control of potentially hazardous energy associated with the Harrop Kiln. Hazardous energy sources included, but were not limited to, thermal energy and electrical energy. c. On or about December 1, 2020, and at times prior, employees performed maintenance and servicing on a diaphragm pump. Machine-specific procedures were not developed, documented and utilized for the control of potentially hazardous energy associated with the diaphragm pumps. Hazardous energy sources included, but were not limited to, electrical, hydraulic, and pneumatic. d. On or about October 7, 2020, and at times prior, an employee performed cleaning of the dies on the Stokes R4 Part-Revolution Mechanical Power Press (MPP). Machine-specific procedures were not developed, documented and utilized for the control of potentially hazardous energy associated with the Stokes MPP. Hazardous energy sources included, but were not limited to, electrical and gravitational. e. On or about October 1, 2020, and at times prior, an employee performed a tooling change on the Stokes R4 Part-Revolution Mechanical Power Press (MPP). Machine-specific procedures were not developed, documented and utilized for the control of potentially hazardous energy associated with the Stokes MPP. Hazardous energy sources included, but were not limited to, electrical and gravitational.
