29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. a. 116 Lehigh Drive Fairfield, NJ, 07004 On or about 9/10/2020, employees engaged in demolition operations to remove 1500 lb. concrete wall panels were exposed to struck-by and crushing hazards when one panel fell in the opposite direction as expected. Neither the engineering survey nor the demolition plan accounted for the direction the structure(s) would be falling towards, which resulted in an uncontrolled collapse of an unsupported concrete wall panel. The wall panel fell in the opposite direction as planned and struck the scissor lift, causing it to topple with an employee in the basket.
