29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary: a. On or about August 27, 2020, at the jobsite, three employees were working in a trench that measured 6 foot 3 inches deep and the spoil pile that measured 4 feet 6 inches high was placed directly along the west wall of the trench. There was also a shovel handle hanging over the edge of the north wall and a small metal pipe next to the edge of the trench, thus exposing the unprotected employees to struck-by hazards.
