29 CFR 1926.1203(a) Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter and evaluates any permit required confined spaces. a) At the Bayside Center 1 by the two 4 stop elevators: On or about July 31, 2020, employees were required to work in the middle of the elevator shaft between elevator A and B where the confined space was not evaluated, exposing them to crushing hazard and struck-by hazards.
