29 CFR 1910.252(a)(2)(iv): The individual responsible for authorizing cutting or welding operations did not designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit: a) On or about, July 28, 2020 Chicago Boiler Company dba CB Mills Division of Chicago Boiler Company, the employer did not ensure that the entry conditions were verified and met, or complete a written permit, prior to employees performing welding inside tanks. In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.
