29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 30, 2020, at a worksite located at 519 Erie Street, Painesville, OH 44077, during residential roofing activities on a steep sloped roof, employees were exposed to a fall of approximately 15 and 10 feet. The employees were using an extension ladder to access the Equipter trash receptor unit and then another extension ladder to access the roof. Neither ladder was properly set up and did not extend at least three feet above the landing areas.
