29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) On or about 06/25/2020 - Large Cooler Area, Loader and Maintenance Mechanic employees were exposed to caught-in hazards due to potentially hazardous mechanical (kinetic) energy at the Palletizer #1 and Strapex machines when Lockout-Tagout Procedure Palletizer #1 revision #1.0 dated 10/27/2018 did not clearly and specifically address the scope and rules for activities such as, but not limited to, clearing jams, repositioning product, triggering the gate for the product count, cleaning up spilled product, and replacing the banding materials.
