29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: Production area: On or about 06/17/2020, employees previously trained in fork-truck operations around calendar year 20104 (or earlier) were not provided refresher training every third year, as required. Untrained employees allowed to operate the company's Toyota forklift (model #-8FGCU, serial number 40158) potentially exposed workers in the immediate area, and others to struck-by hazards.
