29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 230 Castlewood Drive, Berlin, CT: On or about June 8, 2020, the employer did not provide a fall protection training program to employees that were exposed to falls. Following a fall of 18 feet that led to the death of an employee, the anchor that was used was photographed at the request of the Berlin Police Department prior to it being removed for examination. The photo shows that the anchor was installed incorrectly in terms of orientation as well as not being secured in accordance with the manufacturer specifications.
