29 CFR 1915.74(b): Gangways meeting the requirements of 29 CFR 1915.74(a)(1), (2), (9), (10), and (11) were not provided for access from wingwall to vessel, or were not provided when two or more vessels, other than barges or river towboats, were lying abreast. F/V Cascade Mariner, gangways - Two instances of inadequate access were noted: a) Gangway leading up to wing wall - The gangway had shifted and was not level. A top rail obstructed access to the wing wall and a section of the left side ascending standard railing was missing. There was a gap in excess of 1 foot between the gangway and the two step landing which was icy and had no railings. Employees are exposed to slip and trip hazards and fall hazards in excess of 8 feet. b) Gangway from wing wall leading to main deck of vessel - The gangway overhung the bulwark and was not equipped with a safe means of accessing the vessel; a snow/ice covered plastic tub was provided for a landing. The distance between the end of the gangway and the deck was 50"; the distance between the end of the gangway and the plastic tub was 32". Employees are exposed to slip, trip and fall hazards.
