29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: On or about 2/28/2020, at 2810 NE 29th Street Lighthouse Point, FL - An employee was exposed to an electrocution hazard while standing in water and using a handheld drill that was attached to an outlet wired in a manner that reversed the designated polarity for the outlet.
