29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: a) Sion Farm Commercial Center St. Croix, VI - On or about 2/4/2020, the forks on a Sky Track Tele Handler forklift Model No. 10054, Serial No. 16569, with a 10,000 LBS Rated Load Capacity was modified by making a 1 inch round opening on the tip of each tine, exposing employee to a struck-by hazard. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c).
