29 CFR 1910.146(d)(1): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not implement the measures necessary to prevent unauthorized entry: On, or about February 20, 2020, at 8101 Union Ave., CLEVELAND, OH, 44105 in the hazardous materials bay, Kenan Advantage Group failed to implement measures needed to prevent unauthorized entry into permit spaces while attempting rescue.
