29 CFR 1926.1053(b)(16): Portable ladders with structural defects were not immediately marked "Defective, Do not Use" and withdrawn from service: a) At the jobsite, a 24-foot extension ladder that had been split into two sections and used to gain access to the excavation was not marked with "Defective, Do not Use" and withdrawn from service, exposing the employees to a fall hazard of approximately 9 feet.
