29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about February 6, 2020 the employer did not develop, document and utilize written step by step procedures for authorized personnel to follow when accomplishing maintenance/servicing work on, but not limited to Press Brakes, Forklifts, Overhead Crane, and Laser Cutters. This exposed personnel to caught-in, crushing, struck by, and burn hazards, and amputation injury.
