29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were maintained in a legible condition: At a worksite located at 17562 Gum Rd, Carthage, MO: Employees were exposed to struck-by, caught under, and crushing hazards. The employer failed to ensure that all markings on powered industrial trucks, including nameplates were kept legible prior to use. a) This was demonstrated on January 28, 2020 when a Toyota power industrial truck (Serial No. 35459) in the Chub Emulsion was missing the control labels for the operating hydraulics that move the forks left and right. b) This was demonstrated on January 28, 2020 when a Clark powered industrial truck (Fork-032) in the Chub Emulsion had a nameplate that was no longer legible. c) This was demonstrated on January 28, 2020 when a Nissan powered industrial truck (Fork-029) in the Chub Emulsion had a nameplate that was no longer legible.
