29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the ground on the east side of the house, and used by employees to access the steep upper roof were only about 18 inches above the steep roof. b) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the ground on the east side of the house, and used by employees to access the steep dormer roof above the front entrance, were only about 4 inches above the roof. c) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the peak of the dormer roof above the front entrance , and used by employees to access the high steep dormer roof above, were only about 4 inches above the roof. d) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the ground at the southwest corner of the house, and used by employees to access the steep upper roof were only about 12 inches above the roof. e) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the ground at the northwest corner of the house, and used by employees to access the Aluminum pik on a ladder jack scaffold were only about 12 inches above the pik. NO ABATEMENT DOCUMENTATION REQUIRED D & M Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1411625, citation number 2, item number 2, and was affirmed as a final order on 10/10/2019, with respect to a workplace located at 28 Ivy Lea, Tonawanda, NY 14223. DM Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1234454, citation number 1, item number 4, and was affirmed as a final order on 10/12/2017, with respect to a workplace located at 33 Sterling Place, Lancaster, NY 14086.
