29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person. On January 10, 2010 on the fourth floor south tower: The needle beam type scaffolding was not designed to hold the weight of the scaffolding, added materials and workers. A scaffold loaded with granite reinforced concrete (GRC) collapsed while three employees used the scaffold to transfer door beams up to the next level. Employees were exposed to fall hazards.
