29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Jobsite: 16 Anderson Rd, Parker Ford, PA, manufacturing plant, dry side - As the controlling employer, employees servicing the Dry Side machine were exposed to struck-by and entanglement hazards. The employer did not ensure that authorized employees affixed lockout and tagout devices to prevent unintentional startup of the equipment prior to clearing jams. Noted on or about 12/13/19.
