29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Ball, Bounce and Sport, Inc. - Dunkirk, Ohio: On or about January 2, 2020, employees were exposed to injury from pinch points and rotating parts in that the mixer was not locked out prior to cleaning activities. The employer did not ensure energy control application steps to control the electrical and pneumatic energy sources were implemented prior to employees accessing the hazard area to clean pigment and polymer from the mixer's basin the employer did not: - Turn off or shut down the equipment. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished. b. Ball, Bounce and Sport, Inc. - Dunkirk, Ohio: On or about December 20, 2019, employees that entered the oven and cooling chambers of the 120 mold machine were exposed to struck-by hazards in that the machine was not locked out prior to cleaning activities. The employer did not ensure energy control application steps to control the electrical and pneumatic energy sources were implemented prior by employees who accessed the hazard area to clean the employer did not: - Ensure employees had the method or means to control hazardous energy. - Affix lockout device(s) to control the energy source(s). - Verify that isolation and de-energization of the machine or equipment had been accomplished.
