29 CFR 1910.147(c)(2)(iii): Where major replacement, repair, renovation or modification of machine or equipment was performed after January 2, 1990, the employer did not provide an energy isolating device designed to accept a lockout device: On or about November 26, 2019, beneath the deck of a concrete plant, after acquiring and beginning operations at its facility located at 2550 NW 117th Avenue, Miami, Florida, the employer did not ensure that machine and/or equipment that operated the retractable boot was designed to accept a lockout device to prevent exposure to crushing injures when performing residual concrete removal work.
