29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Employees exposed to cave-in and struck-by hazards. At the jobsite located at 507 S. Harrison Hugoton, KS: On or about December 16, 2019, employees engaged in plumbing repair/replacement activities inside of a trench were exposed to cave-in and struck-by hazards in that the employer had not trained all employees to recognize and avoid unsafe conditions associated with trenching and excavations and the regulations applicable to the work environment.
