Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the employer did not ensure that employees were trained in the correct use personal fall arrest equipment at the job site. A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet. The lifeline system being used was not designed for use as a PFAS near leading edges. The following condition exposed the employee to falls exceeding 70 feet.
