29 CFR 1910.147(c)(6): Periodic inspection. 29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a. On or about December 12, 2019, the employer had not conducted, and had not certified, an annual inspection of the energy control procedure for the double band machine and associated attachments, such as the picker or bunch and cut attachments. Employees performed covered activities that involved servicing and maintenance, such as blade changes, attachment removal and installation, and unjamming of the bunch and cut attachment. On December 9, 2019, an employee suffered a partial finger amputation while performing servicing activities without lockout/tagout protections.
