29 CFR 1910.178(l)(3)(i)(G): Fork and attachment adaptation, operation, and use limitations; a. On or about January 14, 2020, and at times prior, the employer's powered industrial truck training content did not cover all of the necessary topics applicable to safe operation of the powered industrial trucks in the workplace. The employer had not trained all employees who operated powered industrial trucks on fork and attachment adaptation, operation, and use limitations when utilizing forklift extensions and other attachments [per 1910.178(l)(3)(i)(G)]. Employees were thereby exposed to hazards such as, but not limited to struck-by, caught-against, struck against, and tip-over.
