29 CFR 1910.334(c)(3): Test instruments and equipment and their accessories were not rated for the circuits and equipment to which they were connected: On or about November 3, 2019, on north exterior side of the facility where an employee was exposed to electrical hazards after unlocking a switchgear enclosure and testing 13,800 volt exposed phases inside the switchgear without using testing equipment that was rated for the circuits being tested.
