29 CFR 1910.27(b)(2)(vi): The employer did not ensure that each employee using a rope descent system, used a separate, independent personal fall arrest system that met the requirements of subpart I of this part: a. On or about November 7, 2019, both the working line rope and the safety "life" line rope of the rope descent system were attached to the same angle iron stanchion of an air-conditioning mounting bracket on the roof, exposing employees to 120-foot fall hazard.
