29 CFR 1910.119(f)(1)(iii)(B): The employer's Standard Operating Procedures did not include precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment: Avantor Performance Materials/Phillipsburg, NJ/Building 101-Methanol Distillation Process Area: a) On or about October 2, 2019, the employer did not ensure that Standard Operating procedures for the Solvent Distillation System (WI-00244) addressed flash fire hazards. The list of required personal protective equipment in the operating procedure did not include flame resistant clothing. Employees work in a Division 1 Class 2 area and would be exposed to flash fire hazards in the event of a Methanol release.
