29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position:(a) On or about October 16, 2019, employees perform maintenance and servicing on various equipment. Employees were exposed to struck by, caught in and amputation hazards in that: a) Sorter Top Area Access The lock tag out device (metal cord) used was not tight enough to stop other employees to turning on the machine while other work on the equipment.
