29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At a trench located at 5900 Cromwell Marine Creek Rd., Fort Worth, Texas; an employee was exposed to cave-in hazards while looking for a 4 inch fiber optic line in an approximately 5-foot 5-inch deep, 54.5-inch wide and 12-feet long trench with a depth with vertical walls on all sides. The employee was exposed to cave-in and crush-by hazards.
