29 CFR 1910.119(d)(3)(i)(B): The employer did not include all equipment that was part of the process on piping and instrument diagrams: At the worksite located at 2504 Industrial Drive, Kirksville, Missouri: Employees were exposed to hazards of ammonia exposure when a release occurred on October 2, 2019, and October 3, 2019. The piping and instrumentation diagram (P&ID) were not accurate with the installation of piping and instrumentation at the facility.
